Because building trust with our patients is an important part of ensuring we’re planning for success, we don’t lure families in with a low price, only to surprise them with add-on fees. Instead, we want you to know exactly what your treatment fee includes.
Initial Exam & Diagnostic Records
- Free comprehensive orthodontic examination*
- Preliminary Diagnosis and explanation of problem
- Preliminary treatment plan
- Study models of teeth
- Facial and oral photographs
- Diagnostic, full-head (cephalometric) x-ray
- Diagnostic dental/jaw (panoramic) x-ray
- Diagnostic functional evaluation
Comprehensive Treatment (Approximately 2 years in many cases)
- All materials and supplies
- Doctor and staff time
- Correspondence with other doctors
- Correspondence with insurance carriers including filing necessary claims
- All progress diagnostic records as required
- Treatment progress reports to parents and dentist
- Kozak Cash incentive program
- 24-hour emergency care for orthodontic appliances
Retention Treatment (Until Phase 2 Begins)
- Removal of orthodontic appliances
- Retention of diagnostic records (as needed)
- Construction of one set of original retainers
- Review of treatment results and future orthodontic needs
Retention Treatment (Approximately 1 year)
- All doctor and staff time
- Retainer adjustments (as needed)
- Progress reports and correspondence to dentist
Insurance Information
Our office is committed to helping you maximize your insurance benefits. Because insurance policies vary, it’s always a good idea for you to become familiar with your benefits. Located on your insurance card, you will find a toll-free number to contact your carrier and determine your benefits.
We will also contact your insurance company to verify benefit amounts and payment schedules. Once treatment has begun, we will follow up with your insurance company to ensure they are paying properly over the course of treatment.
